This policy applies to the Mountain Vista Foot & Ankle website. It does not replace the Notice of Privacy Practices you receive as a patient, which governs how your medical information is handled under HIPAA. You can request a copy of that notice at either office.
Our appointment request form asks for your name, phone number, an optional email address, your preferred office, whether you are a new or returning patient, scheduling preferences, and a brief note about what brings you in. You choose what to share. Please do not include detailed medical or insurance information in the form. We will gather anything we need by phone.
We use form submissions to contact you about scheduling and to prepare for your call or visit. We do not sell your personal information, and we do not use it for third-party advertising.
Like most websites, our hosting platform may log standard technical details such as browser type, device, and pages visited. This information helps keep the site working properly and securely.
This site links to outside services such as our patient portal, online bill payment, and Google reviews. Those services have their own privacy policies, and any information you provide there is governed by them, not by this policy.
This website is not directed to children. Appointment requests for minors should be submitted by a parent or guardian.
